Make plans for a free sports physical for next school year!

The physicals will be held at Corbin High School from 9:00 AM – 1:00 PM on the following dates (see flyer).

Students must complete as much of the KHSAA Athletic Participation Form (GE04) as possible prior to attending. Attached is the link to the KHSAA form.
