📣 Medora Athletes – Sports Physical Announcement! 🏀⚽🏈

Nurses Direct Connect will be on campus on Thursday, April 23rd to provide low-cost sports physicals for Medora students. This is your child’s ONE opportunity this year to complete a sports physical with us. Please plan ahead! 📅

💲 Cost: $35
The fee will be waived for students who:

Have ACTIVE Anthem Hoosier Healthwise, or

Are completing their Annual Well Child Check (WCC) with us.

Your insurance will cover one Well Child Check per year. We can bill for this visit if your child is not seeing a separate pediatrician for their yearly checkup.

Please sign up online at www.NursesDirectConnect.com/home and select the Medora option.

📄 Important:
Complete all paper forms and send them back with your student no later than April 17th. We cannot see students without completed paperwork.
