PHS FALL Student-Athletes and Families:
Cleared/ Not Cleared messages sent again. If you received a NOT Cleared email, your registration is not complete and is missing one, two or three of the required document uploads. Assumption of Risk, Physical and Insurance Card MUST be uploaded. Paper copies/ emails are not accepted.
Please review your account and upload any missing documents to complete the registration process. Your student-athlete cannot participate in practices, tryouts or games until they are cleared.

Registration lists were sent to coaches. If you have not heard from a Coach, the Coach Directory can be found on the Athletic Website {link in bio}

Physical forms can be obtained via your pediatrician's office and must be uploaded along with a copy of your insurance card.

IMPORTANT!!!! No matter what grade the student-athlete is in, an Assumption of Risk form needs to be uploaded. Copies for Student-Athletes in grades 7,8,10,11,12 can be accessed via your Arbiter account from last year.
