Across all of the scientific literature, the best predictor of results from our workout is not the number of sets, reps, type of equipment, concentric or eccentric movement speed, or days per week…the best predictor is whether or not we do it for the long haul.

In other words, the strength workout that we actually stick with is the workout that will produce the greatest benefit in terms of strength, performance, appearance, and health promotion.

The most comprehensive scientific paper on the topic of GLP-1 drugs and weight loss was published last month in the journal Pharmaceuticals.

This 52-page document introduces a paradigm shift in obesity treatment. Instead of focusing on losing weight, the authors introduce “High-Quality Weight Loss”—the notion that we need to be concerned not just about weight loss, but also what is lost.

The authors state: “The future of obesity treatment lies not in greater weight loss per se, but in achieving better weight loss—defined as metabolically favorable, functionally responsible, and clinically sustainable.”

We should focus on losing fat while trying to maintain as much lean muscle as possible. Yes, this is obvious, but it becomes more important with the advent of GLP-1 drugs as significant weight loss has become much more achievable.


3 key take-home messages for those on a GLP-1 or considering a GLP-1:
1️⃣ Focus on more protein (aiming for 0.7 grams of protein per pound of body weight daily) and quality of protein. The authors state, “High-quality sources such as dairy proteins, eggs, fish, lean meats, soy, or targeted oral supplements become particularly useful when total intake is constrained.”

2️⃣ Resistance exercise must be prioritized. “If high-quality weight loss is to remain more than a conceptual aspiration, then resistance training has to be treated as a central therapeutic tool rather than as an optional lifestyle accessory.” The authors go on, “Resistance training improves not only the amount of tissue preserved, but also the usefulness of that tissue.”

3️⃣ Build the reserve. Weight loss induced by a GLP-1 will impact an active, strong, 35-year-old differently than a sedentary 65-year-old. The 65-year-old might be starting the GLP-1 journey with very low muscle reserve. Having less muscle to begin with means that they have less to lose. The authors argue that intense strength training is even more important for those individuals who are starting GLP-1 therapy with low reserves.

Worried about strength training when you have pain like a sore shoulder or aggravated Achilles tendon? It’s tempting to stop strength training that area altogether, but instead try this tactic on a strength training machine:

1. Identify a range of motion that you can move to with no pain
2. Instead of lifting and lowering the weight, simply hold it in that pain-free position for 45-90 seconds
3. Over time you can increase the weight
4. Eventually you’ll reduce the weight and return to a range of motion (lifting and lowering the weight)

The first consideration when selecting a workout (strength training or not) should be: Can I do it when I’m injured and when I’m ill (because we all will get injured and get sick)?

Authors (a collaboration of the brightest 34 scientists from around the world) of the landmark 2025 publication, “Global Consensus on Optimal Exercise Recommendations for Enhancing Healthy Longevity in Older Adults,” published in the Journal of Nutrition and Healthy Aging, assert that the most important time for us to engage in strength training is when we are injured or sick.

When we experience an injury, and the injury causes us to become more sedentary (and stop resistance training), and thus, we lose muscle rapidly. This level of strength and muscle becomes our new baseline.

It’s not the time to stop your workouts. It’s the time to modify. This is the time when you need to get (or stay) strong.

Caveat: This isn’t suggesting you should battle through the flu, a cold, or an acute illness to get your workout in (you should take a few days off in scenarios like these).
