A 2026 cardiology study found heart scarring in recreational marathon runners that shocked researchers.
The marathon has been sold for decades as the ultimate achievement of cardiovascular fitness. Finish a marathon, the story goes, and you have proven your heart is strong. Millions of ordinary people have accepted this without question.
But then a landmark study published in the Journal of the American College of Cardiology in 2026 found something that challenges this completely.
Recreational runners who had completed five or more marathons were showing significantly elevated rates of myocardial fibrosis, which is scar tissue on the heart muscle itself. They were also showing higher rates of coronary artery calcification and atrial fibrillation compared with age-matched non-runners.
These are not the findings you expect in a group of people doing everything right.
What makes this more striking is the contrast at the center of the data. Moderate exercise, defined as 30 to 60 minutes of activity five days per week, remains strongly cardioprotective. The risk curve does not go up gradually. It bends sharply at a specific threshold.
This is what researchers call a J-shaped relationship between exercise and cardiac risk. Light to moderate exercise dramatically reduces the chance of heart disease. But when training volume pushes far beyond recommended guidelines, something measurable starts to go wrong.
James O'Keefe, a preventive cardiologist at Saint Luke's Mid America Heart Institute, has been tracking this pattern for over a decade. His research, originally published in Mayo Clinic Proceedings, found that chronic excessive endurance training can cause structural remodeling of the heart and large arteries, including fibrosis and stiffening of the atria and right ventricle.
Separate research confirmed that endurance athletes have roughly four times the risk of atrial fibrillation compared with the general population.
The body appears to treat extreme endurance workload as a form of long-term stress that leaves physical marks on cardiac tissue.
None of this means that moderate running is dangerous. The data continues to show that it is one of the most protective habits a person can develop. But the finish line of a marathon may not be the health milestone millions believe it to be.
