Here is some info regarding physicals for all of our Merino Jr/Sr High Athletes.
Here is what Banner is offering and the dates!

It's that time of year to get your kids' sports physicals done. Our clinics have set aside some time to help you get these completed.
July 20th 1p-7p
August 3rd 1p-7p
Call (970) 521-3223 to schedule a visit for your child.

$25 if not schedule as well child exam and/or not run through insurance*

Payment due at time of appointment

*If eligible, appointment can be scheduled as a well child exam that includes a sports physical, which will be billed to insurance and is typically covered at 100% by most insurance plans. If the patient prefers not to bill insurance or does not have insurance coverage, we can provide a sports physical for only a $25 fee that is due at the time of service

Any child under 18 must have a parent present.
