Trigger finger isn’t a problem with the tendon itself.

The tendon glides through a series of pulleys (ligaments) that keep it close to the bone. When the pulley becomes thickened or inflamed, the tendon can no longer glide smoothly, causing the painful catching, clicking, or locking sensation many patients experience.

In this ultrasound exam, you can see the tendon moving dynamically beneath the pulley and the exact point where motion becomes restricted.

One of the advantages of musculoskeletal ultrasound is the ability to identify the problem in real time while the finger is moving.

This patient had chronic medial knee pain caused by a painful osteophyte along the inside of the knee, creating constant irritation of the overlying tendons. Instead of repeatedly masking symptoms with injections, we performed an office-based Tenex procedure to precisely remove the offending osteophyte under real-time ultrasound guidance through a minimally invasive approach. The exact pain generator was identified and treated directly. Quick recovery. Immediately ambulatory after the procedure.

This patient had chronic insertional Achilles tendinopathy with painful calcific osteophytes, or bone spurs, at the tendon insertion. She was unable to work.

Using the Tenex procedure, the diseased tissue and bone spurs were precisely removed under real-time ultrasound guidance through a minimally invasive approach.

Chronic Achilles pain is often driven by structural pathology like this, and identifying the exact source matters.

I treated a 42-year-old musculoskeletal provider with over 5 years of persistent left neck and shoulder pain.

She had already seen multiple providers, including specialty care, and had undergone extensive physical therapy and chiropractic treatment without lasting relief.

On exam, her pain localized to the trapezius, scalenes, and sternocleidomastoid regions.

Using ultrasound, I performed targeted ultrasound-guided fascial hydrodissection to address the involved structures.

She had near complete relief immediately following the procedure.

This is not uncommon when the underlying pain generator is accurately identified and treated directly.

We’ll follow her progress and consider further treatment, including PRP if needed.
