Sports Physicals and School Physicals/Immunizations are being offered at the locations below. If your child will be playing sports or if they need a physical or immunizations for school, these services are available. The clinics are for those without a regular health care provider or those who do not have health insurance and will be at no cost to the family.
Students who receive any vaccinations will receive a $20 Visa gift card (first come, first served basis).
All vaccines required for school which include: MMR (Measles, Mumps, Rubella), Varicella (Chickenpox), DTaP (Diphtheria, Tetanus, Pertussis), Tdap (Tetanus, Diphtheria, Pertussis), Td (Tetanus, Diphtheria), Hepatitis A, Hepatitis B, Men ACWY (Meningitis), and IPV (Polio) will be available at the clinics.
COVID-19 bivalent vaccine is also available.

Parents/caregivers must fill out and sign consent paperwork before physicals can be completed. The forms can be found on the JCPS website at: https://www.jefferson.kyschools.us/student-support/health/screenings

Per Kentucky regulation, JCPS Health Services is required to offer various screenings to students.
