Perimenopause is more than a change in your menstrual cycle. It is a physiological transition that can affect your brain, heart, sleep, mood, energy, and ultimately how you feel and perform every day.

Estrogen doesn’t simply disappear overnight. During perimenopause, hormone levels can fluctuate significantly, which is one reason symptoms can feel unpredictable, from brain fog and difficulty concentrating to disrupted sleep, anxiety, changes in your cycle, and heart palpitations.

And while these symptoms can be associated with perimenopause, we should never assume that every symptom is hormonal.

Thyroid dysfunction, iron deficiency, anemia, nutritional deficiencies, and other medical conditions may produce similar symptoms and should be appropriately evaluated by a healthcare professional.

This is also why exercise during this stage of life should be more intentional than simply “working out.”

GLP-1 medications have a legitimate place in medicine, and for the right patient they can be an incredibly valuable tool.

My concern is not appropriate medical treatment; it’s how quickly we are normalising these medications for people who simply want to lose 10–20 pounds for aesthetic reasons.

We spent years trying to move away from “skinny at all costs,” yet somehow being smaller is becoming the goal again, even when it may come at the expense of muscle, strength, performance, and long-term health.

And let’s be clear: just because a medication is FDA-approved doesn’t mean it has no side effects or risks. FDA approval means a medication has been evaluated for specific uses and that its benefits are considered to outweigh its known and potential risks for the intended population. It does not mean the medication is risk-free or appropriate for everyone.

If you’re a medical professional recommending GLP-1s to people without a medical indication simply to help them lose 10–20 pounds, I believe you’re not doing your job right. A prescription shouldn’t replace an honest conversation about nutrition, resistance training, muscle preservation, lifestyle, risks, and whether medication is actually necessary.

Especially after 40, the goal shouldn’t simply be to weigh less. We need to protect the tissue that helps us stay strong, capable, and independent as we age.

There’s a big difference between treating a medical condition and medicating the desire to be smaller.

When Nazik first came to us, she was dealing with significant knee pain and was worried about what the future would look like.
Through proper strength training, individualized programming, and consistency, she gradually rebuilt her strength, improved her confidence, and returned to doing the things she loves without the limitations that once held her back.

One of the biggest myths we hear is that an injury automatically means you have to stop exercising. In reality, most non-surgical injuries don’t require complete inactivity, they require smart modifications. If your knee hurts, there are still countless ways to train your upper body, core, balance, and overall fitness. If your shoulder is injured, there are often safe lower-body exercises you can continue doing. Exercise is not all-or-nothing.

At The Foothills Gym, we work closely with physical therapists and use evidence-based strength training to complement, not compete with, the rehabilitation process. Our goal is to help you stay active, maintain your strength, and continue progressing safely while your body heals.
