Two weeks away!!!

MUSC will be providing FREE sports physicals at AJHS on Friday, May 15th beginning at 2 pm in the high school gym.

Here's what you need to do....
1. Register! See the link below. (select AJHS for the school student attends.)
2. Go by the office to get a Sports Physical Packet.
3. Complete all forms as noted below and RETURN them to Mr. Hanna or Nurse Reeves ASAP.

Each of these forms should be Signed and Dated by both Parent and Athlete
MUSC will complete the Physical Exam form
Physical History
Parent Permission Form
Concussion Awareness Form
Emergency Contact Form
SCHSL Waiver Form
