Family Practice of Holyoke is offering Sports Physicals to help student athletes get cleared quickly and conveniently.

•Dates: June 10 and July 24
•Time: 1 to 3 pm
•Cost: $20 during these event dates
•Location: Family Practice of Holyoke
1001 E. Johnson Street, Holyoke, CO 80734
•No appointment needed
•Parents must be in attendance
•CHSAA Sports Physical form must be completed by parents

Sports physicals are required for school sports participation, and we are here to make the process simple for families.

Looking to save time?

You also have the option to schedule a well-child visit at the same time. This allows you to complete both your child’s annual exam and sports physical in one visit.

Well-child visits must be scheduled in advance by calling 970-854-2500.

Cannot attend these dates?
Sports physicals are available year-round for $30 outside of event dates.

We’re thrilled to introduce our second 2026 O.P.E.R.A.T.E. Scholarship winner in Colorado! These awards hold a special place in our hearts because they are our own signature scholarships, given to athletes who truly O.P.E.R.A.T.E. with our Seven Definitions of a Positive Athlete in every aspect of their lives.

Please congratulate Teagan Martin from Holyoke Senior High School! Teagan founded Running 4 Reasons, becoming the youngest person to finish the Leadville 100 ultramarathon and raising over $13,000 for schools, cancer patients, literacy programs, and adaptive sports. His drive to help his community is unmatched: he co-created a free tax prep service for low-income families, co-founded a lawn care business at just 13, and serves as VP of both FBLA and FFA! This fall, Teagan is heading to CU Boulder's Leeds School of Business to study finance. We love seeing young leaders stepping up and making such a massive impact!

Positive Athlete Names “Most Positive” Athletes in Colorado
for the 2025-26 School Year
DENVER, CO (May 7, 2026) – For the second consecutive year, the Positive Athlete organization is
recognizing local high school athletes from across the state of Colorado as high-character kids who have been
nominated and confirmed by their schools as the most representative of “How Positive Athletes
O.P.E.R.A.T.E.”.

This year, Positive Athlete is naming 28 different Colorado high schoolers as the “Most Positive” male and
female student-athletes across the state, while also recognizing with two coaches, one athletic director, one
athletic trainer and three schools as “Most Positive” within their respective areas. The multiple of 7 theme
stems from the number of attributes (7) called out above.
More than 920 nominations from coaches, athletic directors, athletic trainers, principals, teachers, guidance
counselors, and parents were submitted for the Positive Athlete program state-wide, representing 292 different
public and private high schools (equivalent to 78% of all Colorado high schools with formalized athletics
programs—the highest such percentage in the country). Nominations covered all 27 state sanctioned sports as
well as e-sports, archery, climbing, cycling, equestrian, martial arts, powerlifting, rugby, Unified Sports, and
more. The Colorado awards are part of the national expansion of Positive Athlete, which was created by sports
industry veteran Scott Pederson in the early 2010’s as a movement for more positivity in youth sports. In
addition to excellence on the field, each nominated Positive Athlete was required to show the above
characteristics in order to be nominated.
