150 minutes a week was never the goal. It is the least anyone could prove was worth doing.
The number reached public health guidance because it was the smallest amount of exercise whose benefit showed up clearly across large populations, and because it looked achievable to people currently doing nothing. It was never the point where benefit stopped growing, and nobody claimed it was. But a minimum printed on a poster reads like a finish line, and most people treat it as one.
A 2026 study put a figure on what sits above it. Researchers took 17,088 people from UK Biobank, average age 57, strapped a movement tracker to each wrist for seven straight days, and had them ride a stationary bike to estimate their fitness, meaning how much oxygen the body can use at full effort. Hospital and death records were then followed for almost eight years. In that time 1,233 people had a heart event. Roughly seven in ten of those were atrial fibrillation, an irregular heartbeat, with heart attacks, heart failure and strokes making up the rest.
Hitting the 150-minute guideline was linked to 8 to 9 percent lower risk, and that held no matter how fit someone already was. Cutting risk by more than 30 percent took roughly 560 to 610 minutes a week, which is nine to ten hours, three to four times the guideline. Twelve percent of the group got there. In between, the return was steady rather than fading: every extra hour a week was worth about another 3 percent, and the benefit had not flattened by the top of what they could measure.
Fitness changed the price rather than the direction. The least fit needed about 30 to 50 more minutes a week than the fittest to reach the same reduction, which the authors call the steeper challenge facing people who are out of shape. For a 20 percent cut in risk, that meant 370 minutes a week for the least fit and 340 for the most.
This study watched people rather than assigning them, and that matters. Someone doing nine hours a week differs from someone doing two in a hundred ways no adjustment can remove. Illness also cuts activity long before anyone is diagnosed, so the sickest people pile up at the low end and make high activity look better than it is. The authors tried to get around that using genetics, comparing people who inherited a tendency toward higher fitness with people who did not, on the reasoning that genes are dealt at birth and cannot be caused by an illness decades later. The answer was awkward for the headline. Inherited fitness was linked to about 21 percent lower risk of heart failure. Inherited tendency toward being active showed nothing consistent. Fitness carried the causal support. Minutes did not. Fitness was also only estimated from a bike test rather than measured directly with a mask, and UK Biobank volunteers are 96 percent white and healthier and wealthier than the country they came from.
A large US cohort found that people doing two to four times the minimum had only a few percent lower death rates than people simply meeting it, with no clear gain above that. A pooling of tracker-based studies found risk dropping sharply from the lowest activity levels and then slowing hard. A 2025 analysis of step counts put the bend between 5,000 and 7,000 steps a day. Most of this literature describes a curve that flattens. This one does not, at least not within the range it could see.
Below 150 minutes, that number is still the thing to chase, and the biggest gains anywhere in this literature sit down there. Above it, the case that the work is finished has now lost its best data. And if you are out of shape, the same protection costs modestly more time, which is a reason to start rather than a reason to wait.
