It is never too early to get your student athlete ready to hit the field, court or track for the upcoming season!

Join us for a one-day-only event offering FREE sports physicals for school-aged athletes:

📍 Five Oaks Medical Clinic – Enter through the main doors

No appointment needed—just walk in! Please note, a parent or guardian must be present with children who are not currently patients of Five Oaks. Shot records will be required of non-patients as well.

Whether your child plays football, cheers, runs track or participates in any school sport, this is the perfect time to complete their required physicals at no cost to you. We’re proud to serve our community and surrounding areas and help set our local athletes up for a safe, healthy and successful summer and school year!
