What's wrong Wednesday!

I have taken apart and fixed many over under shot guns but this is a first. I have never seen a hammer "peel" like this. Unfortunately, this company hasn't made shotguns or parts since the 80s and because it is an obscure brand, finding the parts needed is near impossible. The customer declined further repair as the firearm is not worth the cost to make new parts.

What do you think caused this damage?

What's wrong Wednesday!!
This semi auto 12 had a 20g loaded in the tube, so when the customer racked it and nothing happened, the racked it again successfully chambering a 12 behind a now barreled 20. You can see the end result here.
The force also cracked the forearm and stock while breaking the bolt as well.
