Your MRI is not a pain predictor.

Even in high-level athletes.

People assume “findings” only stay silent in sedentary knees — that the second you train hard, the pain will show up.

The data says otherwise.

Major & Helms (2002) MRI’d asymptomatic Duke basketball players.

74% of knees already had structural change — bone marrow edema, tendon signal, cartilage findings.

Zero pain. Zero missed practices. Zero missed games.

Pappas et al. (2016) used 3.0-T MRI on asymptomatic NCAA D1 basketball players.

100% of knees had at least one verified finding: fat pad edema, tendinopathy, bone marrow edema, cartilage change.

Still no pain.

So the scan can look “bad” while the athlete keeps playing.

Treat the patient.

Not the MRI.

Hitting a 300lb deep squat 6 months after fully tearing his ACL?

What about 10 reps with a 210lb seated calf raise?

5lb eccentric Nordics?

Yeah Nick Ryan did all of that ✅

He is also:

1.) Jumping HIGHER than he did before his injury; he is performing East Bay dunks with EASE!

2.) Returning to competitive basketball; scoring >30 points in his first return to the hardwood!

3.) Passing ligamentous tests for stability for his knee!

…all that is left is for MRI confirmation of healing for his fully torn ACL!

It’s not a 100% guarantee… but ACL tears can heal WITHOUT surgery!!!

How do we know?

Studies showing them reattaching under the right tension — proven on repeat arthroscopy (Ihara 1995, Kurosaka 1998).

With an early bracing protocol, 69% of tears are oriented to achieve MRI-verified reattachment 90–100% of the time (Ihara 2017).

When that happens, function can match or even beat ACL reconstruction (Filbay 2022).

In the Cross Bracing Protocol study? 90% MRI-verified healing (Filbay 2023).

Systematic reviews?
• 9/9 studies show the ACL has the capacity to heal
• No difference in return-to-sport rates or activity levels between rehab and surgery

So… ACL tears can heal without surgery!

Do webpages on ACL rupture management actually present evidence-based information?

What does the science say?

A 2022 study by Gamble et al. asked exactly this:

How accurately is the current evidence being shared — and made accessible — to the everyday person searching for help after a full-thickness ACL tear?

Here’s what we do know from the evidence:

    1    You can return to some form of sport without surgery even after fully tearing your ACL

    2    Only ~2/3 of people return to their pre-injury level of sport after ACL reconstruction

    3    25% of people who return to pivoting/cutting sports after reconstruction will experience another ACL tear

    4    Osteoarthritis risk is similar between those who have a reconstruction and those managed non-surgically

Now look at how well websites communicate this:

• Only 48% of webpages mention that return to sport without surgery is possible

• Only 4% mention that just 2/3 of people get back to pre-injury sport levels after reconstruction

• Only 23% mention the risk of re-injury after reconstruction

• 23% falsely claim that ACL reconstruction reduces the risk of osteoarthritis

This highlights a clear need for better patient resources and decision aids that honestly present both the benefits and the harms.

Full Thickness Thursday 🔬

ACL healing without surgery.

Brendan Ruh (Brendan Ruh) attained MRI-verified re-attachment of his fully torn ACL in just 3 months.

He was surfing and doing plyometrics only 2 months into rehab.

UCLA is now conducting a case study on his results.

He tested innovative approaches — BPC-157, BMAC, and PRP — under board-certified, fellowship-trained orthopedic surgeon and sports medicine physician Dr. Kerisimasi Reynolds (Masi Reynolds).

Is this a fluke? Here’s the data you need:

• 2017 study on Type 1–4 ACL tears: 69% of tears heal 90–100% of the time with an early weight-bearing bracing protocol.

• 2023 secondary analysis of the KANON trial: 50% of the time with exercise alone, the ACL can re-attach without surgery — with function levels rivaling ACL reconstruction.

• Cross Bracing Protocol: healing rates reach 90% — right on par with the Ihara data.

Systematic review evidence shows no difference in return-to-sport rates between rehabilitation and reconstruction.

A study on a cohort of high-level athletes (judo, soccer, MMA) reported 77.4% returning not only to sport, but to pre-injury levels without re-injury.

This is not a medical guarantee for every tear. It is clear evidence that non-surgical healing is possible — and often highly effective.

We have utilized ATG Exercise and ATG For Coaches training principles throughout his recovery; heavy emphasis on HIGH LOADING with:

1.) Seated Calf Raise: @ 190 lbs x 10 reps
2.) Poliquin Step: @ 75lbs x 20 reps on 6” box with SB
3.) Front Squat with CHAINZ: @ 180lbs/205lbs w/chainz x 3 reps

…coupled with an emphasis on PATIENCE in desire to return to sport; literature suggests that the safest timeframe for RTS being ~8-9 months — patient goals factor into any pursuit towards faster re-introduction with disclosure of risks involved 🤝

Did you know the ACL has TWO distinct bundles with completely different functions — and that targeted exercise can actually improve them?

• Anteromedial (“blue”) bundle → primary stabilizer during deep knee flexion

• Posterolateral (“red”) bundle → primary stabilizer during shallow knee flexion

The deep-flexion (AM) bundle tears ~20% more often… yet AI estimates only 10% of athletes progressively overload deep knee flexion.

Solution: scalable, progressive deep-flexion resilience work
1 ATG Split Squat — 12 reps @ 3330 tempo
2 Press-Up Slant Squat — 10 reps @ 31X1

New study (just over a month old) reveals precisely how the ACL adapts — and its anatomy is key.

The ACL consists of two distinct bundles:
• Anteromedial (AM) — primary resistor of anterior tibial translation
• Posterolateral (PL) — primary provider of rotational stability

AM is maximally tensioned in deep knee flexion.
PL is maximally tensioned in shallow flexion positions.

This has direct implications for exercise prescription:
→ Soleus + hamstring loading to offload the AM bundle
→ Controlled deceleration work to protect the PL bundle

Notably, the AM (deep-flexion bundle) tears at a ~20% higher rate than the PL. (Chang et al, 2013)

Long-term protection therefore requires progressive exposure to both deep knee flexion tasks and shallow-flexion deceleration — dosed appropriately to your current phase.
