🚨 FOOTBALL PAPERWORK TURN-IN & EQUIPMENT HANDOUT 🚨

📍 AMS Parking Lot

Please bring:
✅ Player Agreement (must be handwritten — no typed signatures)
✅ Copy of Birth Certificate
✅ Final Report Card OR Greene County Auditor Document
✅ Signed Lindsay’s Law Form
✅ Current Sports Physical
✅ Signed Parent Code of Conduct
✅ Emergency Medical Form
✅ Medication Form (if applicable)
✅ State of Ohio Concussion Form

At the trailer:
✔️ Turn in paperwork
✔️ Receive your practice wristband

At the equipment shed:
✔️ Pick up equipment

📣 Cheer families with a football sibling may turn in cheer paperwork during their football family’s assigned time.

The season is getting closer! All Beavercreek tackle football players are scheduled for GCYFL Weigh-Ins on Sunday, August 2 at 4:15 PM at Lakota East.

This is a mandatory league event and a critical step before we kick off the 2026 season. Let’s make it a smooth day by arriving prepared and on time!

Get ahead of the game by checking these items off your list:

✅ Download and complete all required forms (available on the EBFC website under “Required Paperwork”)

✅ Schedule your athlete’s sports physical

✅ Print your athlete’s final 2025-2026 report card from ParentSquare

The paperwork turn-in date is still TBD, but getting these items completed now will help you avoid the last-minute scramble.
