- Current age
- Parent/Guardian name (first & last)
- Parent email
- Parent phone #

There will be a parent meeting during placements to go over all information (cost, tentative schedule, competition info, etc).
Paperwork will be completed at placements. All individuals must sign a waiver to participate. The waiver must be signed by the parent/guardian. We do NOT offer digital forms at this time.
