A. 1 photocopy of Diploma/ Permanent Record/ TOR
B. 2 photocopy of Birth Certificate ( PSA) or Marriage Certificate- if married
C. Barangay Clearance
D. Medical Certificate ( Fit to Train)
E. Filipino Citizen
F. at least 18 years old
G. at least high school graduate, junior high completer, or ALS program graduate
H. Completed training in SMAW NC II or a holder of SMAW NC II
I. Can Communicate both Oral and Written
J. Physically and mentally fit
K. Can perform Basic mathematical computation

Training Hours: 8 AM to 5 PM
Training Duration: Vary according to the prescribed number of hours per Qualification
