📍 Venue: Barangay Mabolo Gym

🛏️ BEDRIDDEN BENEFICIARY
-DSWS Authorization Form – accomplish using BLUE BALLPEN
-Photocopy of Claimant’s Government ID with signature
-Photocopy of Senior Citizen ID of the beneficiary with signature
-Whole-body picture of the beneficiary with a September 2026 calendar visible in the photo

-DSWS Form for Deceased Beneficiary
-Death Certificate
-Photocopy of Senior Citizen ID
-Photocopy of Claimant’s Government ID with signature

-Senior Citizen ID with signature

IMPORTANT REMINDER:
Para sa mga adunay pangutana bahin sa payout o adunay mga requirements nga kinahanglan i-comply, mahimo mo nga mobisita sa DSWS Labangon Office o sa Office of Gender and Development (GAD) sa atong Barangay Hall para sa assistance ug clarification.

Palihug pagdala sa gikinahanglang dokumento ug mag-follow sa mga instructions sa payout personnel.

✅ Physically and mentally fit
✅ With good driving record
✅ Responsible, disciplined, and a team player
✅ Willing to serve and respond during emergencies
