Reporting: Your First Line of Defense

Hazard reporting is one of the most critical components of a proactive safety culture. It allows us to identify and fix dangerous conditions before they lead to an injury or incident.

Why Report Hazards?

Preventative Action: Identifying a "near-miss" or a broken tool today prevents an ER visit tomorrow.

Continuous Improvement: Reporting trends helps management invest in better equipment or more effective training.

Empowerment: Every person on-site has the authorityβ€”and the responsibilityβ€”to speak up when something doesn't look right.

What Should You Report?

A hazard is anything with the potential to cause harm. This includes:

Physical Hazards: Frayed electrical cords, slippery floors, or blocked fire exits.

Unsafe Acts: Someone working at height without a harness or operating machinery without a guard.

Environmental Hazards: Poor lighting, excessive noise, or lack of proper ventilation.

Near-Misses: Events where "nothing happened," but easily could have (e.g., a heavy tool falling but missing a worker).

How to Report Effectively

To ensure a hazard is addressed quickly, follow these four steps:

1. Identify
Notice the hazard and, if safe to do so, mark the area to warn others.
2. Correct
If it is a simple fix (e.g., picking up a trip hazard), do it immediately.
3. Notify
Inform your immediate supervisor or the safety officer right away.
4. Document
Fill out a hazard report form with the location, time, and description of the risk.

The Golden Rule: If you see it, you own it. Don't assume someone else has already reported it.
