Plantar fasciitis ( Diagnosis , Treatment )

Plantar fasciitis is a degenerative condition of the plantar fascia that results in heel pain with symptoms ranging from mild to severe.
It is slightly more common in women than men with peak incidence at age 40-60 years. It is reported to account for 11%-15% of all foot symptoms requiring professional care among adults.
The exact cause is unknown but a common theory is repetitive microtrauma from prolonged walking or running.

Evaluation

The diagnosis should be made based on the clinical exam; the following history and physical exam findings may be used to help
support the diagnosis:
plantar medial heel pain (worse after a period of inactivity and with prolonged weight-bearing)
heel pain following recent increase in weight-bearing activity
tenderness and pain with palpation of proximal insertion of plantar fascia
positive windlass test
negative results on tests for posterior tarsal tunnel syndrome
limited active and passive talocrural joint dorsiflexion range of motion
abnormal foot posture (such as pes planus)
heel pain in nonathletic person with high body mass index

Management

Initial treatments:

Acetaminophen or nonsteroidal anti-inflammatory drugs (NSAIDs) are often used to reduce pain in patients with plantar fasciitis; for mild to moderate pain, acetaminophen (up to 4 g/day) is oral analgesic of choice due to adverse effects associated with NSAIDs.
Use plantar fascia-specific and gastrocnemius/soleus stretching for short-term (1 week-4 months) pain relief. Heel pads may increase the benefits of stretching (Strong recommendation).

Taping:

Use antipronation taping for immediate (up to 3 weeks) pain reduction and improved function (Strong recommendation).
Additionally, elastic therapeutic tape applied to gastrocnemius may be used for short-term (1 week) pain relief (Strong recommendation).
Use manual physical therapy (joint and soft tissue mobilization) procedures to treat deficits in lower extremity joint mobility and calf flexibility in order to decrease pain and improve foot function in patients with plantar fasciitis (Strong recommendation).
Treatments for persistent pain:
Use a 1- to 3-month program of night splints for patients with plantar fasciitis who consistently have pain with first step in the morning (Strong recommendation).
Corticosteroid and other local injections:
Intralesional corticosteroid injections are typically mixed with local anesthetic and injected using the medial approach.
They are not typically recommended as a first-line treatment in patients with plantar fasciitis as the benefits have not been shown to offset the potential harms, including long-term disability (Weak recommendation).
May be indicated for patients with success with NSAIDs or cortisone injections elsewhere in the body.
Iontophoresis and phonophoresis:
Consider iontophoresis with acetic acid or dexamethasone for short-term (2-4 weeks) pain relief and improved function in patients with plantar fasciitis (Weak recommendation).
Consider phonophoresis with ketoprofen gel to reduce pain in patients with plantar fasciitis (Weak recommendation).
Consider platelet-rich plasma injections to reduce persistent plantar fasciitis pain.
Extracorporeal shock wave therapy (ESWT) may also be considered for patients with chronic plantar fasciitis not responsive to other nonoperative interventions.
Reserve surgical plantar fascia release for patients with severe symptoms refractory to at least 6-12 months of conservative management.
