Best alternative pathways for an IMG in US :
1. Pharmaceutical & biotechnology industry
This is one of the strongest alternatives for an IMG who wants to continue using their medical background without completing residency.
Possible positions include:
* Medical Affairs
* Medical Advisor
* Medical Science Liaison
* Clinical Development
* Clinical Research
* Drug Safety/Pharmacovigilance
* Medical Reviewer
* Regulatory Affairs
* Medical Information
* Clinical Trial Management
* Scientific/Medical Communications
For someone with an MBBS/MD plus U.S. healthcare experience, this can be considerably more realistic than trying to obtain a physician position without residency.
2. Clinical research
Universities, academic medical centers, CROs and pharmaceutical companies employ foreign-trained physicians as:

* Clinical Research Scientist
* Research Physician
* Clinical Research Coordinator
* Medical Researcher
* Clinical Trial Physician

Some research institutions are accustomed to sponsoring international professionals, particularly for specialized positions.
3. Clinical Documentation Integrity / physician advisor work
This is particularly interesting for physicians who already have U.S. healthcare experience.

* Denials/Clinical Validation physician

However, the visa issue is the major obstacle. Many CDI employers do not sponsor H-1B for these positions because they can hire U.S. nurses, coders, pharmacists, physicians, etc.
So CDI can be an excellent career alternative, but it should not automatically be considered a reliable visa-sponsorship pathway.
4. Medical affairs
Medical affairs can be an excellent bridge between medicine and industry.
Typical progression:
IMG → Clinical Research/Medical Reviewer → Medical Affairs → Medical Advisor → Senior Medical Advisor/Medical Director
The higher-level roles can potentially support an immigration case based on the individual’s specialized expertise, publications, leadership and national impact.
5. Pharmacovigilance / Drug Safety
This is another strong IMG pathway.

* Pharmacovigilance Physician
* Medical Reviewer
* Safety Scientist
* Signal Detection
* Clinical Safety
* Benefit-Risk Assessment
A physician’s understanding of pathology, pharmacology, adverse events and clinical decision-making can be very valuable.
6. Clinical informatics / healthcare technology
An IMG can move toward:
* Clinical Informatics
* EHR optimization
* Clinical decision support
* Healthcare AI
* Clinical data science
* Medical AI validation
* Clinical terminology
* Health-tech product management
* Physician informatics
This becomes particularly attractive if you add skills such as SQL, analytics, AI/ML concepts, Epic/Meditech expertise or clinical data standards.
7. Academic/research route → O-1 or EB-1A/EB-2 NIW
For an IMG with publications, research, presentations, citations, peer review, leadership or significant professional accomplishments, immigration can potentially become more important than finding a traditional employer-sponsored job.
An O-1 can be available to individuals who demonstrate extraordinary ability, while EB-1A is the permanent-residence counterpart for individuals who meet the higher extraordinary-ability standard.
An EB-2 NIW can also be relevant for professionals whose proposed work meets the national-interest requirements. USCIS specifically has a physician NIW category, although the physician-specific NIW has substantial clinical-service requirements and should not be confused with every nonclinical EB-2 NIW case.
