A trainer’s job is not to simply stand there and count reps. Counting reps is easy. Paying attention is the real job.
A good trainer observes the client throughout the set — their form, breathing, tempo, fatigue, and most importantly, how intensely they are experiencing the exercise.

That is where RPE (Rate of Perceived Exertion) becomes important.
The trainer should ask, assess, and adjust when necessary to make sure the client is getting the right amount of training stimulus — not too little, not unnecessarily too much.
A client should never feel like they are training alone while a trainer is merely counting in the background.
Be present. Be observant. Coach the client — don’t just count the reps. That is the difference between supervising a workout and actually training someone.
