Martial Arts Practice or Procedure and alternative:

Authorize instructor JITEN CH DEKA to perform the Practice or procedure of Shaolin Kung-Fu Martial Arts and also give my consent for such practice / Procedure , Which may be found necessary during the course of practice.

1. Risks: The authorization is given with understanding that any practice or procedure involves risks and hazards including unintentional human error. The more common risks include injury, bleeding etc.These risks can be serious enough and may be fetal . There may be some other complications during immediate and recent practice period and or some remote complications, which are inherent to practice procedure.

2 . Additional procedures: If my instructor discovers a different unsuspected condition at the time of practice, I authorize my instructor to perform additional procedure he thinks necessary.

3. I understand that no guarantee or assurance has been made of the result of the procedure.

4. No one can predict that risks will or will not happen in any case.

5. Student / Guardian consent: I have read and fully understood this consent form and concerned instructor has explained to me in language which I understand. I give my consent for the practice procedure, after understanding and considering all the facts mentioned above.
