Indian doctors rarely check for this.
Because nothing looks wrong.
Normal weight.
Normal BMI.
Normal fasting sugar.
Clean chit every time.
Yet beneath that lean frame, visceral fat builds quietly around organs, driving hidden damage.
WHAT IS TOFI:
TOFI stands for Thin Outside, Fat Inside.
It describes people who look slim but store dangerous fat around their liver, pancreas, and heart.
Unlike pinchable fat under your skin, visceral fat pumps inflammatory chemicals straight into your bloodstream.
You can't see it.
You can't feel it
Routine checkups miss it completely.
WHY INDIANS ARE VULNERABLE:
South Asians are genetically prone to storing visceral fat at much lower body weights.
An Indian at 60kg can carry the metabolic risk of a Westerner at 85kg.
This is why Indians develop diabetes and heart disease younger and at lower BMIs.
Staying slim while eating junk often just means storing fat internally.
SILENT SIGNS:
β Hard, firm belly rather than soft fat
β Post-meal fatigue
β Fasting sugar creeping toward upper limits
β Family history of diabetes despite being lean
β Elevated triglycerides
HOW TO ACTUALLY CHECK:
β Waist circumference (>90cm men, >80cm women)
β Waist-to-height ratio (waist should be under half your height)
β Fasting insulin & HbA1c (flags early insulin resistance)
β Liver ultrasound (screens for fatty liver)
THE FIX:
β Strength train 3x week - builds muscle, reduces visceral fat specifically
β Walk daily - 8,000-10,000 steps reduces visceral fat more than any other lifestyle change
β Increase protein - 1.6-2g per kg bodyweight
β Reduce refined carbs and sugar - primary driver of visceral fat accumulation
β Get the right bloodwork - don't rely on weight or BMI alone
THE TRUTH:
Normal weight and fasting sugar aren't enough.
Share this with the thin person in your family who thinks they're safe.
They deserve the full picture. πͺ
