Your spine says one thing. Your hip says another.

So which T-score should you believe?

Discordance between spine and hip T-scores is actually quite common — and it’s one reason I don’t like looking at a DXA number in isolation.

Different skeletal sites contain different proportions of trabecular and cortical bone and may change at different rates. Age-related changes, hormones, medication, previous injuries and even technical factors can also influence what we see.

So if your results don’t “match”, don’t panic — get curious.

Depending on your situation, additional information such as **TBS, Z-scores or REMS (Echolight) may help add context to the DXA findings.

Because the goal isn’t simply to improve a number.

It’s to understand your bones well enough to make better decisions about how you protect your strength, mobility and independence.

The biggest mistake I see people making on GLP-1s?

Watching how much weight they're losing without asking **what they're losing.**

With significant weight loss, some lean mass can be lost alongside fat.

Why does that matter beyond metabolism?

Because MUSCLE AND BONE ARE A TEAM!

Muscle creates forces on the skeleton. Stronger muscles support movement, balance and the mechanical loading that helps maintain strong bones.

So when I'm thinking about GLP-1-assisted weight loss, I'm not only thinking:

“How much weight did you lose?”

I'm thinking:

How much muscle did you preserve?
Are you getting stronger?
And what are we doing to protect your bones in the process?

GLP-1 medications can offer important metabolic benefits for appropriate people. This isn't about demonising them.

It's about making sure that in pursuit of a lower number on the scale, we don't neglect the tissues that help keep us **strong, mobile and independent for decades.**

One place to start: progressive resistance training.

I’m fascinated by the science of healthy ageing — but I also think we’re in danger of overcomplicating it.

We talk about hormones, mitochondria, peptides, biomarkers and the latest optimisation tools.

All fascinating. But there’s a more fundamental question:

If you’re losing bone, muscle, strength and physical capacity, what exactly are you optimising for?

This is why I talk about structural longevity — building and maintaining the physical foundation that allows us to remain strong, mobile and independent.

And that foundation doesn’t exist in isolation. Sleep, metabolism, inflammation, stress, nutrition, bone and muscle all influence each other.

The goal isn’t simply more years. It’s having the capacity to live those years well.

That’s Strong Aging.

5 things I would stop doing after 50.

Not because 50 is some magical biological deadline — it isn't.

But because as muscle, hormones, bone turnover, recovery and metabolic function change, some strategies that appeared to work earlier in life may no longer be enough.

❌ Relying on cardio alone
Cardiovascular fitness matters, but it doesn't replace the stimulus muscle and bone receive from progressive resistance and appropriate loading.

❌ Eating less to stay slim
Maintaining a low body weight isn't the same as maintaining a strong body. Protecting muscle and meeting your nutritional needs become increasingly important.

❌ Normalising poor sleep and chronic stress
These don't just affect how tired you feel. They interact with glucose regulation, appetite, recovery, inflammation and hormonal signalling.

❌ Treating every symptom separately
Bone loss, changing cholesterol, increasing abdominal fat, poor sleep and fatigue can appear to be completely different issues. Sometimes the important question is: what connects them?

❌ Assuming every change is simply ageing
Age is a risk factor. It isn't always the underlying explanation.

This is why I don't believe healthy ageing is about collecting more supplements or chasing individual symptoms.

It's about understanding the system that's changing — and knowing where to intervene.

What worked at 35 may not be enough at 55.

That doesn't mean your body has failed you.

It means the strategy needs to evolve.

Strong Aging is about building strength, resilience and independence for the decades ahead.

Having osteoporosis doesn't mean every symptom you experience is caused by your bones!

Fatigue and breathlessness are two good examples.

Osteoporosis is usually silent until a fracture occurs. But some of the factors contributing to low energy — nutrition, thyroid or hormonal issues, poor recovery or an underlying health condition — may also matter for your bones.

That's why I don't look at osteoporosis in isolation.

And if breathlessness is new, unexplained or persistent, please have it medically assessed rather than assuming it's “just osteoporosis.”

A diagnosis should never stop us looking for the rest of the story.

Save this if you're trying to understand the bigger picture behind your bone health.

When people see me using red light, they often assume it’s for my skin.

But that isn’t the main reason I use it.

Red and near-infrared light—known as photobiomodulation—may influence mitochondrial activity and support circulation, tissue repair and exercise recovery.

But red light is not an anti-ageing miracle. Its effects depend on the wavelength, dose, distance and what you are trying to support.

For me, it’s an additional recovery signal—not a replacement for movement, nutrition or good sleep.

Before using a device, check its actual wavelengths and recommended dose. A red glow alone tells you very little.

Having osteoporosis does not mean you need to give up yoga.

Yoga can support balance, posture, mobility and body awareness—all important for staying strong and reducing falls.

But certain movements may need adapting: deep forward bends, deep or forceful twists and headstands, particularly with spinal osteoporosis or previous vertebral fractures.

The aim is not to become afraid of movement. It is to make the movement appropriate for your body.

Before your next class, tell your instructor about your bone health and ask for osteoporosis-safe modifications.

Resistance training deserves its place in a bone-health programme—but it isn’t the only kind of movement that matters.

Bone strength is important. So is being able to balance, react, coordinate your movements and remain confident in your body.

Yoga and Pilates may not provide the same progressive bone-loading stimulus as resistance training, but they can develop physical qualities that weights alone may not train sufficiently.

This isn’t about choosing one over the other. They have different roles.

Keep your resistance training and add one weekly yoga or Pilates session focused on balance, posture and controlled movement.

Your hip shows osteoporosis, your spine shows osteopenia—and an X-ray reveals arthritis.

It is easy to assume that the spine must simply be stronger.

But that may not be the full story.

Arthritis and other degenerative changes can sometimes make spinal bone density appear artificially higher on a DEXA scan. The result may look more reassuring even though it does not accurately represent the strength of the bone itself.

This is why I never look at one T-score in isolation.

A DEXA measures bone mineral density. An X-ray provides different information about the structure of the spine and joints. Each test answers a different question—and the real value comes from interpreting the findings together.

Your scan gives you a number. Proper interpretation gives that number meaning.

Practical step: Obtain copies of both reports and ask for them to be reviewed together.
