Here are the instructions to fill out the new NTP Sports form!
Coaches, schools & Little League, please share to your parents!
DO NOT ADD THE $5 SITTING FEE!
It is now included in the package prices!
Minimum is still $25.00!
How to Fill Out Your New Sports Photo Order Form
1. Fill Out the Top Information
* Name of Guardian: Parent or guardian’s name.
* Team Name: School/team (example: Healdton Lady Bulldogs Softball).
* Phone #: Best contact number.
* Email: Required if ordering any digital images.
* Player’s Name: Exactly how you want it to appear on the photo.
* Jersey #: Player’s jersey number.
2. Choose a Package
Minimum order is $25.00.
Select one of the package options or build your own order.
Package A – $25
* 1 Printed 8x10
* Choose either:
* Individual with Team
* Individual Only
Package B – $25
* 2 Printed 5x7s
* Choose 2:
* Individual 5x7
* Team 5x7
Or one of each
Package C – $25
* 1 Printed Individual 5x7
* Plus 2 add-ons of your choice:
* 8 Wallets
* Button
* Magnet
Package D – $25
* Choose 4 add-ons total:
* 8 Wallets
* Button
* Magnet
(You may repeat items if desired.)
3. Build Your Own Package
If you prefer, you may purchase items individually by writing the quantity next to each item.
Any of the items listed below may also be added to any package if desired.
* Printed 8x10 Individual with Team – $25
* Printed 8x10 Individual – $25
* Digital 8x10 Individual with Team – $20
* Digital 8x10 Individual – $20
* Printed 5x7 Individual – $15
* Printed 5x7 Team – $15
* 8 Wallets – $10
* Button – $10
* Magnet – $10
Remember: Your total order must be at least $25.00.
4. Calculate Your Total
Add up your order and write the total in the “Total Paid” box.
5. Select Your Payment Method
Check the appropriate box:
* Cash
* Check
* Credit Card (add $3 processing fee)
* Venmo
* Cash App
If your payment is made under a different name than the guardian listed on the form, please write the name the payment was sent under.
