Good morning parents,
Starting this school year, RHS and RMS will be moving to a platform called Healthy Roster for preseason document collection and signing, and injury tracking and communication. I am asking all parents to complete this 2 minute Google Form so I can send you the information to create your Healthy Roster account for your student athlete(s). If you have multiple children at RHS or RMS, please complete the Google Form multiple times so I can create each individual account. I have attached a link that answers the majority of the questions you will have and you can ask me directly as well. Healthy Roster can be used on a computer or downloaded as an app.
Once your account is created, please go into details and update your insurance, emergency contact (email and phone number), student demographics, and any other medications/allergies that your student athlete(s) may have. If you don't have insurance, please look at RHS Activities website for supplemental insurance. Under the documents tab, you will find all the preseason documents that must be completed before your student athlete(s) can compete. These forms can be digitally signed on your phone. Your athlete's physical can also be uploaded directly through the app by taking a picture of BOTH sides of the physical form. Please check to make sure you have all your signatures! All concussion testing is TBD as this will now be a requirement for both the high school and middle school.
Turn on your notifications with the app as I will send notifications if your preseason documents are missing information and needed to be updated.
