Milford Youth Football & Cheer (MYFC) is a 501(c)(3) non-profit organization based in Milford, Massachusetts, dedicated to promoting youth participation in football and cheerleading. Operated entirely by volunteers, MYFC focuses on building character, self-confidence, and good sportsmanship among young athletes.

Tackle Equipment Fittings and Handout will take place at our new location 75 South St Hopkinton MA

If you are unable to attend your Teams Session, there are open house sessions.

PAPERWORK will be accepted during all sessions.

Required Forms - PRINT SINGLE-SIDED

Physical Form - Dated Jan 1, 2026 or Later
If it is dated 2025, submit the MEDICAL CLEARANCE FORM to your child's doctor. This is not to be confused with the RESUME PARTICIPATION MEDICAL CLEARANCE FORM. RESUME form will not be accepted.
Birth Certificate - New Players must provide a copy of their birth certificate.
Copy of June 2026 Report Card - REQUIRED FOR WAIVERS ONLY

JERSEY ORDERING

If you are ordering a new jersey for the 2026 season, please be prepared with payment at the fitting event. Returning athletes have first dibs on their previous season number.

Jersey purchase is NOT required, MYFC will issue a game day jersey for all athletes who choose not to purchase. PRICE $78 TACKLE $63 FLEX

GUARDIAN CAPS

Payment for a Guardian cap is due at the equipment fitting. PRICE $60 limited quantity
