Each student must have their completed paperwork before stepping on the bus to see the provider. Please do not use PanCare Intake forms from previous years, as they have been updated.

Every required parent/guardian signature spot must be signed. The EL2 form has two sections requiring both the parent and student signatures (Pages 2 & 4).

If a student comes to the bus with any portion of the forms incomplete or missing a required parent/guardian signature, he/she may be asked to reschedule if appointments are still available. Students must have the original completed forms to present to the medical team on the bus.

Additionally, If copies are needed, the parent/guardian must complete the Authorization to Release Medical Information form (attached) and email it to [email protected]. Once the completed authorization form is received, please allow up to 30 days for processing. Copies of requested forms will be sent via certified mail.

Shout out to our goalkeeper, AJ Amerantes!

AJ has been selected to represent North Florida in the FACA All-Star Game on March 15–16. This prestigious event recognizes the top senior boys and girls for their outstanding high school seasons and brings together elite talent from the North, South, East, and West regions of Florida.

Being chosen for this game is a tremendous honor. It highlights AJ as one of the best players in the state and gives him the opportunity to compete against top-tier talent. With scouts in attendance, this showcase could open important doors for his future both on and off the field.
