1) Registration Forms with details of all team members
2) Indemnity Forms signed and completed by all team members (Personal Particulars Form Section A)
3) Parental Consent, if applicable (Personal Particulars Form Section B)
4) $20 (Passion Member) or $35 (Non-Passion Member) Cheque Payable to Bukit Batok CC YEC
