1) Registration Forms with details of all team members
2) Indemnity Forms signed and completed by all team members (Personal Particulars Form Section A)
3) Parental Consent, if applicable (

4) $20 (Passion Member) or $35 (Non-Passion Member) Cheque Payable to Bukit Batok CC YEC
