📌 IMPORTANT: No Parent Consent and Waiver Form, No Tryout.

📝 How to Download the Parent Consent and Waiver Form

1️⃣ Scan the QR code to download the Parent Consent and Waiver Form.
2️⃣ Print the downloaded form.
3️⃣ Complete the form by providing all the required information.
4️⃣ Secure the signature of your parent or legal guardian.
5️⃣ Submit the completed Parent Consent and Waiver Form to the Sports Development Office
