除郵寄到香港防癌會辦事處，報名人仕亦可將報名表格連同劃線支票(抬頭：香港防癌會)交到「香港保齡球城」報名表格收集處。

截止日期: 3月10號 (星期六)

Attention please:

NEW APPLICATION COLLECTION POINT (EFFECTIVE FROM TODAY):
In addition to mailing to the HKACS office, applicants can also send your application form together with a crossed cheque made payable to “The Hong Kong Anti-Cancer Society” to Hong Kong Bowling City Application Collection Point.

Application deadline: 10 March (Sat)
