NEW UNIQUE FERTILITY SERVICE
FuturePositive (FP) is offering this service FREE OF CHARGE for those accepted into the programme by the 31st May 2015.
The only charges will be for incidental expenses used in the provision of the service. For example, travel or room rental costs etc. All charges will be provided at cost to FuturePositive.
THE SERVICE - THE PROCESS
1. No Medical Issues. The provision of the FP service is reserved for those who have already undergone medical tests that confirm there are no medical reasons why either the man or woman in the relationship can have children. Only healthy individuals should apply for this service.
In order to be accepted onto the programme you will have to provide written confirmation from your GP that both of individuals in the relationship have no medical reasons for infertility.
2. An initial Pre-Service meeting with the couple which may last between 30 minutes and an Hour to discuss the couples history and introduce them to the FP service.
Based on the result of the Pre-Service meeting each individual will need to provide extensive information about various aspects of their lives.
3. A series of meetings to run through a comprehensive series questionnaires. These series will be held individually with both partners. There is no set number of meetings as answers to some questions may lead to subsequent questions.
4. The provision of the FP service - these will be created based on the answers to the questionnaires - the solutions are different for different individuals.
NOTE:
Couples will be required to sign a non-disclosure Agreement and a Separate Provision Agreement.
