First 60 Seconds with Your Patient

As a Band 6 nurse on an acute ward, if I’m not in charge, I’m caring for 9 patients. The start of my shift is everything. After handover, before the ward becomes busy and unpredictable, I complete my first 60‑second check with each patient.

This isn’t a full assessment. It’s a RAPID
Introduction 👋
A visual review 👀
And a safety check ✔️.

In those first moments, I’m looking at breathing, colour, alertness, and overall presentation. I’m also noting who may need an immediate observation check, who looks stable, and who might need prioritising as the shift unfolds.

Most importantly, it ensures every patient knows who I am and feels comfortable asking for help when they need it.

What led to this moment. Past medical history and reason for admission and their diagnosis.

A — Assessment 💙
What you think is going on.
Your clinical judgement, even if you’re still building confidence. Also mention what observations and investigations you have done already such as News Score, blood glucose, blood culture.

R — Recommendation 💚
What needs to happen next.
What you need, what you’re asking for, or what you think should be done.

SBAR isn’t about sounding perfect, it’s about sounding structured, even on the days when you’re tired, overwhelmed, or second guessing yourself 🤔.
It gives you a framework to lean on so you don’t freeze, ramble, or forget key details.

The shift that shook me ended up shaping me.

A comment that stung pushed me to grow, learn, and become the nurse I needed to be for my patients.

I didn’t wait for confidence, I built it.

Face‑to‑face training, online courses, YouTube tutorials, learning from experienced colleagues and every professional who walked onto our ward. I absorbed everything.

One year later, I stepped into the Band 6 role I once needed help from.

Focus on improving yourself, your skills, and your knowledge. The rest will fall into place.

So many new nurses go through moments like this. The pressure is real.
